8/11/2023 EnchantaVaughn Approved Behavior: QMHPA arrived to the home upon arrival to the home of Ms. Broadnax. Upon arrival to the home, Ms. Broadnax appeared to be in a foul mood evident by her facial expressions as well as Ms. Broadnax engaging in general conversation with QMHPA. Ms. Broadnax’s home appeared to be unorganized as well as Ms. Broadnax’s overall appearance. QMHPA engaged Ms. Broadnax in a discussion to determine how her day was. Ms. Broadnax verbalized to QMHPA that her day was not going to best and that she is just tired of people. Ms. Broadnax did not present any symptoms of SI or HI at the time of the session being conducted. Intervention: QMHPA began the session by inquiring about Ms. Broadnax’s current thoughts, feelings and emotions. QMHPA educated Ms. Broadnax on appropriate means of maintaining appropriate personal hygiene, and clean living quarters. QMHPA educated Ms. Broadnax on how allowing her hygiene to be neglected as a symptom of her depression. QMHPA educated Ms. Broadnax on the negative health factors related to poor personal hygiene. QMHPA assisted Ms. Broadnax in purchasing appropriate cleaning supplies for her home as well as personal hygiene products. QMHPA educated Ms. Broadnax on the importance of setting aside time every day to do basic maintenance so that the task of maintaining a clean home environment would not overwhelm her. QMHPA educated on how and unkempt environment and poor self-care only perpetuates the symptoms of her depression. Upon ending the session, QMHPA created a tentative chore chart in an attempt to remind Ms. Broadnax and assist her in completing daily chores. Response: Ms. Broadnax appeared to be receptive to QMHPAs’ interventions and services that were provided to her during the session. Ms. Broadnax stated that over the course of the last few days she had falling into a state of depression and did not have the energy nor the desire to clean herself or her home. With prompting from QMHPA, Ms. Broadnax was able to empty the trash, separate dirty clothes from clean clothes, and pick up the trash around the home. Ms. Broadnax agreed that she needed to take better hygienic care of herself and be more vigilant of her ADLs. Ms. Broadnax stated to QMHPA that she just didn’t feel like cleaning her home because she was tired and she admits that she needs to do better; however, there are a lot of things going through her mind a minute and it causes her to be depressed and when she gets in that mood she doesn’t want to do anything.
